- All owner contact information, date of birth, and driver’s license number
- Information of the owner of your business property if it is rented
- If applicable, the social security number of owner
- Ownership type: sole proprietor, corporation, trust, partnership, or LLC
- Business name
- Opening date
- Sales type
- Estimated monthly gross receipts
- If applicable: federal employer’s ID number, seller’s permit number, state contractor’s number, and/or state employer ID number
- Physical and mailing address of business
- Signature verifying that all information is true and correct to the best of your knowledge
Do you need any other permits or licenses in Visalia?
