  7. Federal Tax ID number or social security number if you are the sole proprietor

  11. Contact information, birthdate, and valid driver’s license information for all owners

  14. You must sign and date your application. Your signature denotes that all of the information you have provided is true and accurate, that you agree to comply with any and all federal, state, and/or local regulations, laws, and requirements and that you will risk your license being denied or revoked if you are not in compliance or have been found to provide any false information on your application.
