the Department of Revenue online or downloading the
form
D. Obtain State-issued permits and licenses. These

What type of questions will be asked on the
Sales and Use Tax License application?
While filing the Sales and Use Tax License, you can
expect to be asked the following:
• ownership name
• date business first started
• business entity (Corporation, General Partnership,
LLC. Etc.)
• business address
• estimated monthly sales volume
• list of owners names and addresses
