Does Provo Require Specialty LIcenses?
Provo requires certain types of businesses to obtain additional licenses. Provo also requires all businesses to fill out a public safety form. The following list details the different business license applications Provo offers, and the necessary information for each application. Temporary Business: A temporary tax number, business entity number, federal identification number, and event information (location of event, inside/outside event, proposed start and end date, and a description of the event), personal identification, a site plan (for an outside event), a property owner letter (for an outside event), a clean-up bond (for an outside event), and insurance (for fireworks).
Beer License: Ownership and management information, a $1,000 fee for a cash or corporate bond, qualifying information, a list of all employees who will be handling beer, and a Provo Police Department criminal history request form.
Industrial Questionnaire: An SIC number, a description of manufacturing services or activities, a list of raw materials used, a list of chemicals used, a list of principal products, water source, consumption, and treatment information, discharge information, and a plan for spill prevention counter-measures.
Restaurant Questionnaire: A description of food preparation and clean-up activities, an estimate of amount of customers served per day, type of kitchen fixtures used, food disposal information, and grease interceptor/trap information.
Solicitor Permit: Immediate supervisor information, products or service information, a BCI report less than 180 days old, proof of identification, appropriate licenses/permits, and a series of personal history questions.
What Additional Steps Are Required?
If a business is a food establishment, including grocery stores and bakeries, inspections by the Utah County Health Department and Department of Agriculture are required. Further action may be required by the Provo Zoning Department, Fire Department, Police Department, or Water Department during the application review process. How Much Does a Business License Cost?
A commercial business license has different costs based on the number of employees you hire:
0-10 Employees: $150
11-51 Employees: $275
More than 51 Employees: $475
There are also specific fees for businesses operated from home:
