Both the Scottsdale business occupational and professional license application and the Transaction Privilege and Use Tax license are identical in the information that they require.  This information includes the names, contact information, driver’s license information and social security numbers of all owners of your business, the contact phone numbers and mailing and physical address of your business, and a description of your business.  If you rent your business property, you will need to provide the name and contact information of your landlord or property management company. 

In addition, you will need to supply information on previous owners of your business, if it is applicable.  Be sure to submit the ownership type of your business, whether you are the sole owner, if you are involved in a partnership, if your business is part of a larger corporation, a LLC, LLP or another ownership type.  You will need to declare your business type, which could include wholesales, contracting, bar, property rental etc.  Depending on the ownership and business type, you may need to contact the IRS for a FEIN (federal employer’s ID number).  You could also need a state tax ID number from the Arizona Department of Revenue
